Left hand cranks usually fall off if they weren't installed properly from new. Once they come loose and they're ridden for a while loose, the square hole in the crank arm starts to round out. Once this happens, no ammount of tightening will secure it and the only option is a new left hand crank arm. Not expensive at around £15 and maybe LBS. hope this helps, G.
